Key Responsibilities for RN/LPN:

  • Conduct ongoing evaluations of the patient's physical and psycho-social conditions
  • Collaborate with the Interdisciplinary Group (IDG), patient/family, and attending physician to create and implement a care plan
  • Provide education and interventions to manage the patient's pain and other symptoms, while assessing the responses to treatment
  • Ensure availability of necessary equipment, supplies, medications, and services
  • Adjust the IDG care plan as the patient's/family's needs evolve
  • Maintain accurate and timely documentation of services provided to patients and families
  • Participate regularly in IDG meetings

Qualifications for RN/LPN:

  • Active, unrestricted RN/LPN license in the State of Michigan
  • A minimum of one (1) year of relevant nursing experience (required)
  • Preferred experience includes one (1) year in management and one (1) year in home health
  • Current CPR Certification
  • Experience in community health and a broad range of Medical-Surgical care (preferred)
  • Reliable transportation with a valid driver's license and current auto insurance
