About the Role

Kaiser Permanente is seeking a skilled Registered Nurse to join our team in providing high-quality care to patients in need of hospice services.

Key Responsibilities
  • Assess patient needs and develop individualized care plans to ensure optimal comfort and health.
  • Collaborate with interdisciplinary teams to coordinate care and ensure seamless transitions.
  • Provide education and support to patients, families, and caregivers on hospice care and the dying process.
  • Supervise and document Hospice Aide care delivery to ensure high-quality care.
  • Participate in department quality management and improvement activities to drive continuous improvement.
Requirements
  • One year of medical-surgical or ICU nursing experience in an acute care setting, ambulatory nursing, or home health, hospice, or palliative care.
  • Two years of hospice experience (within the last 5 years) as an LPN may be substituted for one year of nursing experience.
  • Baccalaureate degree in nursing or commensurate knowledge and experience.
  • Certification in Hospice and Palliative Care Nursing.
  • Operational knowledge of computerized clinical documentation and office application systems.
Work Environment

This role requires a reliable automobile to travel to and from patient homes. The ideal candidate will have a strong understanding of hospice care and the dying process, as well as excellent communication and interpersonal skills.
