Job Description:
This role offers an exciting opportunity to make a difference in the lives of millions of people by engaging members face-to-face and/or telephonically to complete comprehensive needs assessments. As a RN Clinical Coordinator, you will develop and implement person-centered care plans to address medical, behavioral, functional, cultural, and socioeconomic needs.

You will partner and collaborate with internal care teams, providers, and community resources/partners to implement care plans and make appropriate referrals based on care integration needs. Your role will involve providing education and coaching to support member self-management of care needs and lifestyle changes to promote health.

The ideal candidate will have 2+ years of nursing experience, an unrestricted RN license in Louisiana, and intermediate computer proficiency. This position requires reside in the State of Louisiana and ability to travel locally approximately 25% of the time. The estimated salary for this role is $65,000-$80,000 per year, depending on location and experience.
