We are seeking a skilled Nurse Care Coordinator to join our team at Mass General Brigham. This role is an exciting opportunity for individuals who are passionate about delivering high-quality patient care. As a Nurse Care Coordinator, you will play a vital part in ensuring seamless transitions of care and facilitating effective communication among healthcare providers.

About the Role

In this position, you will be responsible for coordinating patient care services, collaborating with multidisciplinary teams, and developing individualized care plans. Your primary focus will be on delivering exceptional patient outcomes, improving quality of care, and enhancing the overall patient experience.

$81,672 per year (32 hours/week) - an additional $1/hour as a member of the Resource Team. We offer a comprehensive benefits package, including medical, dental, vision, life insurance, and employee assistance programs.

Responsibilities
  • Coordinate patient care services, ensuring seamless transitions and effective communication among healthcare providers
  • Collaborate with multidisciplinary teams to develop individualized care plans
