We are currently seeking a Nurse Evaluator to play a vital role in our Patient Care Management team. This full-time position will involve conducting comprehensive assessments in various settings, focusing on the needs of potential members.

Key Responsibilities:

  • Performing in-person evaluations of members in their residences, in collaboration with an Occupational Therapist.
  • Assessing physical and cognitive conditions to identify functional limitations and the potential for benefiting from PCA services.
  • Documenting evaluations accurately and promptly to reflect the member's needs for assistance with Activities of Daily Living (ADLs) and Instrumental Activities of Daily Living (IADLs).
  • Engaging in effective communication and collaboration with Occupational Therapists, Skills Trainers, and other team members to ensure comprehensive care.

Qualifications:

  • Current licensure as a Registered Nurse or Licensed Practical Nurse in Massachusetts.
  • A minimum of two years of nursing experience.
  • Experience in direct care for elderly or disabled adults is preferred.
  • Home-care experience is advantageous.
  • Completion of the Mass Health PCM Training program is required.
  • CPR certification is necessary.
  • Familiarity with community support systems.
  • A valid driver's license and access to a vehicle are required.

Job Type: Full-time

Compensation: $36.00 per hour
