NP HouseCalls Statewide Traveler

Optum is seeking a skilled Nurse Practitioner to join our HouseCalls team in Michigan. As a HouseCalls Statewide Traveler, you will provide annual clinical assessments to patients in the comfort of their homes, helping to identify and reduce health risks and coordinate follow-up care to improve health and well-being.

• Conduct in-home health assessments on enrolled Medicare Advantage members, averaging 45-60 minutes in length.
• Review past medical history, medication, and perform evidence-based practice screenings.
• Identify diagnoses, formulate a list of current and past medical conditions, and communicate findings to inform members' PCP of potential gaps in care.
• Educate members on disease processes, medication, and compliance.
• Intervene in urgent and emergent situations and coordinate follow-up care.

• NP license and national certification (ANCC or AANP) or ability to obtain.
• Family, Geriatrics, Adult, Adult-Gerontology Primary or Adult-Gerontology Acute Care certification.
• Access to reliable transportation to travel to client and/or patient sites within a designated area.

• 1+ years of clinical experience in their highest level of education, clinical setting.
• Experience in gerontology, cardiology, internal medicine, or endocrinology.
• Home Health care or home visit experience.
