Key Responsibilities

Review and analyze medical records to accurately code diagnostic and procedural information in line with national coding standards and reimbursement protocols. Collaborate with healthcare providers to resolve any discrepancies in documentation and ascertain the correct coding assignments. Extract clinical diagnoses and procedure codes, ensuring comprehensive documentation is recorded in the electronic medical records system. Proactively identify and rectify missing information to maintain complete and accurate patient records. Conduct quality assurance reviews of medical records to ensure adherence to coding regulations and standards set forth by relevant regulatory bodies, including state Medicaid plans and Medicare services. Compile and prepare reports as needed, aggregating data from medical records for analytical and research purposes.

All duties must be performed in accordance with established policies, procedures, and regulatory requirements, while consistently delivering exceptional service to all stakeholders.

Required Qualifications

A high school diploma or GED, or equivalent training in medical record management, anatomy, physiology, and medical terminology is essential. An Associate's degree in a related healthcare field is also acceptable.

Candidates must demonstrate proficiency in ICD and CPT coding principles, ideally validated by certification from recognized professional organizations. A minimum of six months of coding experience in diverse healthcare settings is required, along with the ability to meet accuracy standards on coding assessments.

Proficiency in common office and coding software is necessary for this role.

Preferred Qualifications

Certification as a Registered Health Information Administrator (RHIA), Registered Health Information Technician (RHIT), Certified Coding Specialist (CCS), or Certified Professional Coder (CPC) is preferred. Relevant experience may be considered in lieu of formal certification.
