Job Title: Home Health Physical Therapist

LHC Group is seeking a highly skilled and compassionate Home Health Physical Therapist to join our team in Worcester, Massachusetts.

Job Summary

We are looking for a dedicated and experienced Physical Therapist to provide high-quality care to our patients in the comfort of their own homes. As a Home Health Physical Therapist, you will work closely with our interdisciplinary team to develop and implement individualized treatment plans that promote optimal patient outcomes.

Responsibilities
  • Evaluate patients' functional status and physical therapy needs, and develop treatment plans to address these needs.
  • Provide one-on-one physical therapy sessions to patients in their homes, focusing on improving mobility, strength, and overall function.
  • Collaborate with other healthcare professionals, including physicians, nurses, and occupational therapists, to ensure comprehensive care.
  • Document patient progress and communicate with the healthcare team to ensure seamless care coordination.
Requirements
  • Current Physical Therapy licensure in the state of Massachusetts.
  • Current CPR certification.
  • Must have a valid driver's license and access to a reliable vehicle.
