Job Summary

Satellite Healthcare, Inc is seeking a highly skilled Medical Social Worker to join our team. As a Medical Social Worker, you will play a vital role in supporting the social and emotional well-being of patients with kidney disease.

Key Responsibilities
  • Conduct psychosocial assessments to identify patients' needs and develop care plans
  • Participate in care planning and team review to ensure patients receive comprehensive care
  • Provide education and support to patients and their families to enhance quality of life
  • Collaborate with healthcare teams to address patients' social and emotional needs
Requirements
  • Master's Degree in Social Work from an accredited CSWE institution
  • MSW required; LCSW/ACSW preferred (CA); Licensure/LMSW required in states with licensure (TX, TN)
  • One (1) year of work experience in a behavioral or healthcare-related setting, preferred
