We are seeking a Specialty Pharmacist to fulfill a vital role within AndHealth.

Key Responsibilities:

  • Deliver outstanding patient care and support to healthcare providers.
  • Provide clinical pharmacy services as part of the Patient Management Program.
  • Offer specific clinical services through established protocols or collaborative agreements when appropriate.
  • Administer clinical services, therapeutic recommendations, and counseling for patients with chronic conditions, including but not limited to asthma, dermatitis, epilepsy, hyperlipidemia, infertility, Hepatitis B, Hepatitis C, HIV/AIDS, Inflammatory Bowel Disease (IBD), Lupus, Migraine, multiple sclerosis, psoriasis, psoriatic arthritis, pulmonary arterial hypertension, rheumatoid arthritis, and transplant.
  • Regularly review and stay updated on clinical guidelines and literature essential for managing specialty care effectively.
  • Serve as a resource for healthcare providers and patients seeking health and medication information.
  • Conduct thorough reviews of patient therapies to identify therapeutic duplications, drug interactions, and potential allergies.
  • Actively engage in the pharmacy's Quality Management Committee and Quality Management Program.
  • Provide reports on Quality Improvement Projects across pharmacy locations.
  • Evaluate and suggest medication alternatives, consolidation, and dose adjustments in line with current clinical guidelines.
  • Collaborate closely with Engagement Specialists to ensure continuous medication access for all patients.
  • Review insurance formularies and manage prior authorizations and appeals as needed.
  • Document all necessary information in specialty pharmacy software to maintain accreditation standards.
  • Participate in URAC and ACHC specialty pharmacy accreditation processes and visits as required.
  • Mentor pharmacy interns and students.
  • Perform any additional duties as assigned.

Qualifications:

  • PharmD, BS in Pharmacy, or equivalent FPGEC (NABP).
  • Unrestricted pharmacist license in good standing in Ohio.
  • Willingness to obtain and maintain licenses in multiple states.

Preferred Skills and Experience:

  • Minimum of 3 years of direct patient care experience in pharmacy.
  • BLS/CPR certification required (or must be obtained within one month of employment).
  • Immunization delivery certification is preferred.
  • At least 2 years of specialty pharmacy experience is preferred.
  • Completion of a PGY1 residency or equivalent experience is preferred.
  • CSP credential is preferred.
  • Exceptional written and verbal communication skills.
  • Strong attention to detail and ability to manage multiple tasks simultaneously.
  • Proficient with technology and adaptable to new systems.
  • Willingness to travel as required, with reliable transportation.

Health Center Requirements (if applicable):
