Hospice Registered Nurse

We are seeking a compassionate and skilled Hospice Registered Nurse to join our team at Heartland of Perrysburg. As a Hospice RN/ RN Case Manager -PRN, you will play a vital role in providing personalized care to our patients and their families.

Key Responsibilities:
  • Act as primary liaison between physician, patient/family and hospice team
  • Provide direct patient care services to hospice patients and supportive care to the patient's family
  • Empower patient's caregivers by educating them on care for their loved one
  • Manage and collaborate with an interdisciplinary group consisting of Licensed Practical/Vocational Nurses (LPN or LVN), Certified Nursing Assistants (CNA) and support staff
  • Evaluate patient and family needs and assist in the preparation and execution of the plan of care for each patient
  • Maintain all assigned patient clinical records
  • Participate in company quality assessment and performance improvement programs
Requirements:
  • A compassionate and empathetic nature
  • Ability to provide care wherever the patient calls home
  • RN license in the state of requested employment (or eligibility to obtain)
  • 1+ years of nursing experience, preferably in hospice, home health, ICU, geriatric, or oncology
  • Current driver's license and automobile insurance
  • Current CPR certification
