Responsibilities- Deliver exceptional patient care by assessing, planning, and implementing individualized care plans to meet the unique needs of each resident.
- Manage resident care by coordinating with healthcare professionals, families, and other stakeholders to ensure seamless care delivery.
- Monitor vital signs and report any changes or concerns to the healthcare team.
- Administer medications and treatments as prescribed by physicians.
- Maintain accurate medical records by documenting resident care, progress, and any changes or concerns.
- Supervise Certified Nursing Assistants (CNAs) to ensure they are providing high-quality care and support to residents.
- Provide treatments and dressing changes as prescribed by physicians.
- Paid time off (PTO time) earned based on hours worked.
- No-cost telehealth services available 24/7.
- Time and a half pay for some major holidays.
- Competitive hourly rate of $27.60 plus shift differential.
- Valid license and current registration to practice as an LPN in New York State.
