About the RoleAs a Home Health Registered Nurse/PRN, you will provide high-quality, patient-centered care in the comfort of patients' homes. You will work collaboratively with our interdisciplinary team to develop and implement individualized care plans that meet the unique needs of each patient.
Responsibilities- Provide comprehensive nursing care to patients in their homes, as prescribed by physicians in the plan of care.
- Collaborate with the care team to develop and implement patient care plans.
- Manage nursing treatments and activities to meet the needs of each patient.
- Evaluate patient progress and utilization of services, and review with the care team, patient, family members, physician, and case manager as needed.
- Perform PRN visits and be available on call as required.
- Geriatric Home Health experience.
- Texas Nursing License.
- Strong organizational and self-management skills.
- Strong and compassionate customer service skills.
- Valid driver's license.
- Multiple major medical plans to choose from and spousal insurance.
