Job Summary

Aveanna Healthcare is seeking a skilled Physical Therapist to join our team. As a Physical Therapist, you will be responsible for planning, conducting, and implementing physical therapy treatment programs to restore or maintain patients' well-being.

Key Responsibilities
  • Deliver Physical Therapy Services: Comply with prescribed physician orders and provide physical therapy services to patients.
  • Follow Policies and Procedures: Adhere to all applicable patient care standards, licensure regulations, and state laws.
  • Provide Written Evaluations: Define physical therapy needs, goals, progress, and expected outcomes in conjunction with referring physicians' Plans of Treatment.
  • Coordinate Services: Participate in coordination of services, including discharge planning, with healthcare teams, physicians, nurses, and other staff members.
  • Maintain Effective Communication: Communicate effectively with staff, healthcare team members, and family members.
  • Supervise Personnel: Supervise other personnel, such as Physical Therapist Assistants (PTAs) and Home Health Aides (HHAs), as applicable.
  • Complete OASIS Assessments: Complete OASIS assessments where allowed by state professional practice and regulation.
Requirements
  • Education: Bachelor of Science in Physical Therapy from an American Physical Therapy Association-approved program.
  • Licensure: Current, unrestricted state license as a Physical Therapist in the state of practice.
  • Certification: Current CPR certification.
  • Skills and Abilities: Demonstrated proficiency in clinical assessments, documentation, and compliance with policies and procedures.
Preferred Qualifications
  • Experience: One (1) year of recent experience as a Physical Therapist in an acute care or rehab setting.
  • Pediatric Experience: Pediatric experience preferred.
Physical Requirements
  • Language: Must be able to speak, write, read, and understand English.
  • Travel: Must be able to travel.
  • Lifting: Must be able to lift 50 pounds.
  • Physical Abilities: Prolonged walking, standing, bending, kneeling, reaching, twisting; must be able to sit and climb stairs; must have visual and hearing acuity; must have a strong sense of smell and touch.
Environment
  • Work Environment: Must be able to function in a wide variety of environments, which may involve exposure to allergens and other various conditions.
  • Infection Control: Possible exposure to blood, bodily fluids, and infectious diseases.
