MDS Coordinator - RNJob Summary:
Cranberry Place is seeking a skilled MDS Coordinator to join our team. As a key member of our healthcare staff, you will be responsible for assessing and coordinating care for our residents.
Key Responsibilities:
- Conduct Comprehensive Assessments: Conduct thorough MDS assessments to accurately reflect residents' needs and capabilities.
- Analyze Data and Identify Opportunities: Analyze MDS data to identify care plan opportunities and optimize resident outcomes.
- Collaborate with Interdisciplinary Teams: Work with healthcare professionals to develop and implement individualized care plans.
- Ensure Compliance and Accuracy: Ensure compliance with regulatory standards and maintain accurate documentation.
- Utilize Clinical Judgment and Problem-Solving Skills: Apply strong clinical judgment and problem-solving skills to address resident needs.
- Mentor and Educate Staff: Provide guidance and education to staff on MDS processes and regulations.
Requirements:
- Active RN License: Possess an active Registered Nurse (RN) license in the state of Michigan.
- Experience and Knowledge: Previous experience as an MDS Coordinator or in a similar role is preferred. Strong knowledge of the MDS assessment process and RAI guidelines is essential.
- Excellent Communication and Interpersonal Skills: Demonstrate excellent organizational, communication, and interpersonal skills.
- Ability to Work Collaboratively: Ability to work collaboratively with a diverse team.
