Job Summary

As a Registered Nurse in our Specialized Care Home Care team, you will play a vital role in delivering high-quality patient care in a home setting. Under the supervision of the Nursing Manager, you will work collaboratively with the interdisciplinary team to assess, implement, and evaluate patient care plans.

Key Responsibilities
  • Communicate effectively with patients, families, and healthcare team members to ensure seamless care coordination.
  • Assess patient needs and develop individualized care plans to promote optimal health outcomes.
  • Administer medications and treatments as prescribed by physicians, ensuring patient safety and adherence to regulatory guidelines.
  • Educate patients and families on disease management, medication adherence, and healthy lifestyle choices.
  • Collaborate with the healthcare team to identify and address patient concerns, and provide emotional support and guidance as needed.
  • Participate in ongoing quality improvement initiatives to enhance patient care and outcomes.
Requirements
  • Associate's Degree in Nursing (ADN) or Bachelor's Degree in Nursing (BSN)
  • 2 years of relevant experience in home care or a related field
  • Current RN licensure in the State of Michigan
  • Basic Life Support (BLS) certification
  • Ability to lift up to 50 pounds and perform physical demands of the job
Work Environment

This role is based in Grand Rapids, MI, and requires occasional travel to patient homes and healthcare facilities. You will work in a fast-paced, dynamic environment, collaborating with a multidisciplinary team to deliver exceptional patient care.
