RN/LPN Clinical Assessment Coordinator Responsibilities:
- Conduct assessments, Minimum Data Set (MDS), and care plans for all assigned residents.
- Supervise the timely completion of MDSs by other disciplines according to regulatory guidelines.
- Notify supervisors of any incomplete or delayed assessments from non-nursing disciplines.
- Initiate care plans and related activities aimed at achieving optimal outcomes for assigned residents.
- Generate and distribute the monthly care plan calendar for the upcoming month.
- Facilitate care plan conferences for residents under your care.
- Maintain and update all care plans and assessments for assigned residents at least quarterly, adjusting as necessary.
- Review all resident incidents and the daily 24-hour report to ensure care plans reflect current and evolving needs.
- Coordinate the activities of nursing, rehabilitation, dietary, activities, social services, and restorative nursing during patient care conferences.
- Ensure nursing staff completes the 7-day look-back documentation within the prescribed MDS timeframe.
- Participate in facility management meetings as needed.
- Audit clinical record documentation for assigned residents to ensure accuracy and timeliness.
- Engage in ongoing education, both within and outside the facility, to stay informed about current and new regulations.
- Maintain up-to-date knowledge of the MDS assessment process.
Experience with MDS 3.0 Preferred.
