About the Role

We are seeking a skilled Mental Health Practitioner to join our team at Community Care Alliance.

Job Summary:

The successful candidate will be responsible for assessing and monitoring the psychiatric and medical needs of clients referred to BH Link. This will involve providing assessment and crisis intervention, administering medication as per standing and/or prescriber orders, and completing documentation according to BH Link protocols. Additionally, the practitioner will coordinate admissions with referral sources, administer detoxification protocols, and perform medication reconciliation reviews of medical records.

Key Responsibilities
  • Assessment and Crisis Intervention: Provide assessment and crisis intervention to clients experiencing behavioral health issues
  • Medication Administration: Administer medication per standing and/or prescriber orders
  • Documentation: Complete documentation per BH Link protocols
  • Admission Coordination: Coordinate admissions with referral sources
  • Detoxification Protocols: Administer detoxification protocols to include administration of CIWA and COW scales
  • Medication Reconciliation: Perform medication reconciliation reviews of medical record
Requirements
  • RN required
  • Current license to practice in Rhode Island
  • Experience and interest in working with individuals who have psychiatric and/or substance abuse issues
  • Ability to work as a member of a multi-disciplinary team
  • Ability to assess medical and psychiatric needs of all clients admitted to the BH Link
  • 1-2 years' experience working in a healthcare setting
Working Conditions
  • Prepare and administer medications per MD or standing orders
  • Complete comprehensive SA, MH and physical safety assessment on all new admissions
  • Provide direct care services as needed
  • Administer detoxification protocols per standing orders
  • Provide for safe transport of BH Link clients to higher levels of care
