Job Summary:

The Social Worker Case Worker will be responsible for assessing client needs and strengths, creating service plans, and implementing referrals to Agency and Community resources for entitlement programs, employment, and vocational rehabilitation services. This role requires strong communication and interpersonal skills, as well as the ability to work effectively with diverse populations.

Key Responsibilities:

  • Assess client needs and strengths, and develop individualized service plans
  • Provide referrals to Agency and Community resources for entitlement programs, employment, and vocational rehabilitation services
  • Monitor and evaluate client progress, making adjustments to service plans as needed
  • Collaborate with internal and external stakeholders to ensure seamless service delivery
  • Provide exceptional customer service to clients and stakeholders

Requirements:

  • Masters degree in Social Work from a Council of Social Work Education (CSWE) accredited program
  • Valid license as a Graduate Social Worker in the state of Maryland
  • Knowledge of social work principles, practices, and theories, as well as human behavior and development
  • Ability to work effectively with diverse populations, including individuals with mental health and substance abuse issues
  • Strong communication and interpersonal skills
