About the Role

We are seeking a skilled Nurse Practitioner to join our team at Enhabit Home Health & Hospice. As a national leader in home-based care, we are committed to providing high-quality patient care in the comfort of their own homes.

Responsibilities
  • Provide medical care to hospice patients, including history and physical exams, diagnosis and treatment of medical problems, and education and counseling of individuals, families, and groups.
  • Determine hospice eligibility and pain and symptom management.
  • Participate in the care of the hospice patient as a member of the interdisciplinary team.
  • Perform face-to-face patient visits to assist in determining hospice eligibility.
Requirements
  • Must be a graduate of an approved school and licensed as a nurse practitioner in the state in which they currently practice.
  • Must have basic demonstrated technology skills, including operation of a mobile device.
  • At least five years of nursing experience with a minimum of one year in hospice preferred.
  • At least three years of clinical registered NP experience preferred.
