Key Responsibilities:- Conduct thorough in-home assessments, including medical history reviews, medication reconciliation, vital signs, and comprehensive physical exams.
- Perform evidence-based practice screenings, including point-of-care testing as needed.
- Identify diagnoses and develop care management plans to address patient needs.
- Communicate findings with patients' primary care physicians to ensure seamless care coordination.
- Intervene in urgent and emergent situations, providing timely support to patients.
- Educate patients on disease processes, medication management, and compliance strategies.
- Active NP license or ability to obtain by start date, with unencumbered licensure and no open disciplinary actions.
- Active ANCC or AANP national certification in Family, Geriatrics, Adult, or Adult-Gerontology Primary Care, or the ability to obtain certification by start date.
- Ability to complete physical demands of the job, including lifting 30-pound bags, navigating stairs, and performing physical assessments.
- Access to reliable transportation for up to 85% of travel within licensed states.
- Additional licensure in other geographic areas may be requested.
- 1+ years of clinical experience in the highest level of education, clinical setting.
- Experience in Gerontology, Cardiology, Internal Medicine, or Endocrinology.
- Home Health care or home visit experience.
