Job Summary:

We are seeking a dedicated and compassionate Licensed Caregiver - LPN to join our Health & Wellness Team. As a key member of our team, you will provide high-quality care to our residents under the supervision of registered nurses and healthcare professionals.

Key Responsibilities:

  • Assist residents with personal hygiene, including bathing, grooming, and dressing.
  • Support residents with medication administration.
  • Reposition residents in bed and escort them around the community.
  • Provide toileting and incontinent care.
  • Take vital signs, such as blood pressure, heart rate, and pulse.
  • Observe and report on residents' health, escalating any problems efficiently and effectively.
  • Detail daily reports to the nurse or designated charge person at the end of the shift.
  • Offer emotional support and serve as a source of comfort for residents and their loved ones.
  • Recognize abnormal changes in body functioning and report to a supervisor.
  • Perform nursing assessments, including vital signs monitoring, physical examinations, and health status evaluations, to identify and address resident needs promptly.
  • Provide wound care and other nursing interventions as directed by the registered nurse or healthcare provider, following applicable state regulations, established protocols, and infection control practices.

Requirements:

  • Must be at least 18 years old.
  • Must pass State and Company criminal background/drug screens.
  • Demonstrate effective time management skills.
  • High school graduate or equivalent.
  • Must have a valid, unrestricted nursing license in the state.
  • Current CPR certification.
  • Maintain CEU requirements.
  • Read, write, speak, and understand the English language.
  • Make independent decisions when circumstances warrant such action.
  • Interpret and implement written or oral policies, procedures, and instructions.
  • Staff supervision.
  • Assessment and resident care skills.
  • Medication management principles and practice.
  • Written and verbal communication skills.
