Position Overview

As a **Travel Nurse** specializing in **Value-Based Care**, you will play a crucial role in enhancing patient outcomes by coordinating and managing relationships with healthcare providers. Your expertise will be vital in ensuring that patients receive the highest quality of care while meeting essential performance metrics.

Key Responsibilities
  • Develop and maintain strong partnerships with participating healthcare practices.
  • Ensure comprehensive understanding of quality performance measures such as HEDIS and STAR metrics.
  • Implement processes to guarantee that all patients meet established quality standards.
  • Coordinate the delivery of effective, quality-focused healthcare services tailored to individual patient needs.
  • Collaborate with physicians to plan, implement, and evaluate appropriate healthcare services.
  • Utilize clinical expertise to assess and monitor patient care options, facilitating optimal healthcare outcomes.
  • Identify high-risk patients for potential case management interventions.
  • Engage with medical service providers to enhance communication and streamline referrals.
  • Support patients during transitions of care and conduct follow-up outreach as necessary.
  • Participate in interdisciplinary team meetings to discuss patient care strategies.
  • Conduct telephonic outreach or home visits when required.
  • Communicate effectively with various departments to address issues and document trends.
  • Adhere to administrative guidelines and participate actively in staff meetings.
Qualifications
  • Must possess a **Bachelor of Science in Nursing (BSN)**.
  • Three to five years of recent experience in an acute care setting, case management, or utilization management.
  • Experience with HEDIS and/or CMS STARs is advantageous.
  • Bilingual candidates, particularly those fluent in Spanish, are preferred.
  • Proficient typing skills with a minimum of 35 words per minute.
  • Strong knowledge of current healthcare practices and community resources.
  • Ability to travel to various healthcare provider locations as needed.
  • Demonstrated ability to work independently with minimal supervision.
Work Environment

For those working remotely, a reliable internet connection is required, with a minimum speed of 10 Mbps download and 5 Mbps upload.
