About the Role
As a Clinical Pharmacy Leader, you will be responsible for providing expert clinical pharmacy services, including medication therapy management, patient education, and collaboration with healthcare teams. Your expertise will help us improve patient outcomes and enhance the overall quality of care we provide. You will work closely with our interdisciplinary team to develop and implement individualized treatment plans that meet the unique needs of each patient.

Key Responsibilities
- Provide comprehensive medication therapy management services to patients across various age groups and disease states.
- Develop and maintain relationships with patients, families, and healthcare providers to ensure seamless communication and care coordination.
- Conduct thorough medication reviews to identify potential issues and develop strategies to mitigate risks.
- Collaborate with healthcare teams to develop and implement individualized treatment plans that meet the unique needs of each patient.

Requirements
To be successful in this role, you must have a strong foundation in pharmacology, as well as excellent communication and interpersonal skills. You should also be able to work effectively in a fast-paced environment and prioritize tasks efficiently. Additionally, you should be comfortable operating in a collaborative, shared leadership environment where open communication and teamwork are valued.

Salary Range
The estimated annual salary for this position is between $115,000 and $140,000, depending on experience and qualifications.
