We are seeking a skilled and compassionate Registered Nurse (RN) to join our team as a Hospice Nurse Specialist in Catasauqua, Pennsylvania.

Job Summary

In addition to these benefits, we also offer a competitive salary range of $65,000 - $90,000 per year, based on experience, plus mileage reimbursement and opportunities for career advancement.

Responsibilities

As a Hospice Nurse Specialist, you will be responsible for:

  • Providing high-quality patient care and support to individuals with terminal illnesses
  • Developing and implementing individualized care plans in collaboration with the interdisciplinary team
  • Conducting thorough assessments and evaluating patients' conditions, identifying areas for improvement and making recommendations for changes to their care plans
  • Maintaining accurate and detailed records of patient interactions, including progress notes and medication administration records
  • Communicating effectively with patients, families, and healthcare providers to ensure seamless care coordination
Qualifications

To be successful in this role, you must have:

  • A current license as a Registered Nurse in Pennsylvania
  • A minimum of one year of recent, verifiable clinical (medical/surgical) experience
  • Prior home care experience strongly preferred but not required
  • Graduation from an accredited and approved nursing program
  • Ability to work independently and manage time effectively
  • Strong interpersonal skills and excellent communication abilities
