- Continuously assess and update the Physical Therapy care plan.

- Accurately document observations, interventions, and evaluations related to client care management.
Qualifications:
- A valid Physical Therapist license in the state.
- At least one year of recent experience as a Physical Therapist.
- Home health care experience is preferred but not mandatory. Proficient computer skills and familiarity with electronic medical records (EMR) are advantageous.
- Willingness to travel locally to assigned cases.
