About Our Team
Primary Children's Summit Day Programs is seeking a skilled Clinical Mental Health Professional to join our multidisciplinary team in supporting our Partial Hospitalization (Day PHP), Crisis PHP, and Intensive Outpatient (IOP) Programs. This position offers an excellent opportunity for a passionate Clinical Mental Health Professional to work with adolescents aged 12-18 and contribute to improving the lives of struggling teens.
Job Overview
- The Clinical Mental Health Professional will perform psychosocial assessments, develop and implement plans of care, and assess crisis situations to provide needed resources and referrals for patients.
- This role involves working collaboratively with patients, families, healthcare providers, insurers, community resources, and other involved parties.
$34.06-$52.55 Exempt per Hour or $30.94-$47.77 Certified per Hour
Key Responsibilities
- Diagnose and treat various mental, emotional, and behavioral disorders, modifying care plans as necessary.
- Refer patients to other services to improve their well-being, including community services, treatment programs, support groups, psychiatrists, or other mental health providers.
- Work with the multidisciplinary team to develop discharge plans, ensuring continuity of care.
- Comply with federal, state, local, and clinical program/facility standards, acting within the scope of the Mental Health Practice Act.
- Prioritize daily tasks by assessing patient needs and organizing work effectively.
- Provide supportive, crisis, bereavement, and other social work interventions as necessary.
- Educate and communicate with multiple patient care teams and outside agencies to ensure quality care, timely discharges, and resolution of delays and issues.
Mandatory Qualifications
- A Master's Degree in Social Work or related field from an accredited institution, with degree verification required.
- Current State Licensure as a CSW, LCSW, or LMFT, or CMHC.
- Excellent written and verbal communication skills.
- Computer skills, including email and word processing.
Preferred Qualifications
- Experience working with patients, families, and treatment teams.
- Successful completion of the National Counseling Examination (NCE) or the National Clinical Mental Health Counseling Examination (NCMHCE).
Work Environment
- Ongoing need for employee to see and read information, labels, assess patient needs, operate monitors, identify equipment, and supplies.
- Frequent interactions with patient care providers, patients, and visitors requiring verbal communication, hearing, and understanding spoken information quickly and accurately.
- Manual dexterity of hands and fingers to manipulate complex equipment with precision and accuracy.
- Expected to lift and utilize full range of movement to transfer patients and obtain/distribute supplies and equipment.
