Job Summary:

Baylor Scott & White Health is seeking a compassionate and skilled Registered Nurse to join our dynamic Hospital at Home team in College Station, Texas. As a Hospital at Home Registered Nurse, you will have the unique opportunity to provide personalized care to patients within their homes.

About the Role:

As a key player in patient care management, you will work with other healthcare staff to deliver patient care effectively. Our Registered Nurses use the nursing process efficiently, diagnosing patient responses to health issues and creating care plans accordingly. You will prioritize needs, create, implement, and assess care plans to achieve outcomes, adapt plans as necessary, and observe indicators closely to support evidence-based changes.

Responsibilities:
  • Provide high-quality acute care services to patients enrolled in the Hospital at Home program.
  • Conduct comprehensive assessments and develop individualized care plans tailored to each patient's needs.
  • Spend additional time at bedside assessing clinical and environmental needs and supporting an overall healing environment.
  • Act as a key educator and provide ongoing support to patients and their families, ensuring they have the necessary resources and understanding to manage their care at home.
  • Collaborate closely with interdisciplinary teams to coordinate care and optimize patient outcomes.

Qualifications:

We are seeking a Registered Nurse with a minimum of 2 years of experience in a relevant role. You should hold a BLS certificate and a Registered Nurse License. Familiarity with nursing and patient care standards and procedures is essential, as well as awareness of legal requirements and standards of certification and accreditation bodies.
