Healthcare Professional for Home Visits

Role Summary:
We are seeking a skilled Certified Nurse Practitioner (NP) to join our Advantmed provider network. As an NP, you will be responsible for conducting in-home wellness risk adjustment assessments for Medicare members and other at-risk populations. Your primary objective will be to assess the overall health and well-being of member beneficiaries to ensure accurate and comprehensive risk adjustment coding, leading to greater value-based care.
This is a 1099 (PRN) opportunity that offers tremendous flexibility and potential for those balancing competing priorities. Key Responsibilities:
- Perform annual in-home wellness visits and risk adjustment health assessments on populations with chronic conditions, including comprehensive assessments, physical examinations, body system reviews, vital signs, medication history reviews, prescription reviews, functional status evaluations, pain management, cognitive assessments, home environment reviews, fall risk screenings, social assessments, daily living activity reviews, mental health screenings, and documentation.
- Provide individualized quality care to elderly populations in their homes, building relationships through exceptional communication and empathy.
- Deliver patient health education on topics such as pain management and medication.
- Assist in closing quality care gaps, including screenings and labs.
- Contribute to the enhancement of care delivery as an enthusiastic collaborator.
- Commit a minimum of 30 hours per month.
Requirements:
- Valid unencumbered NP License for the state where you will be working.
- Previous in-home risk assessment experience is preferred. Preferred Skills:
- 3 years of patient care experience in adult or geriatric primary care, EMR experience.
- Ability to work within our EMR system independently.
- Strong ability to communicate effectively and build trust with members.
- Bilingual skills are a plus.
