  5. Maintain accurate documentation of services provided and individual progress in accordance with agency standards.
  6. Engage in problem-solving with service providers to address any coordination issues.
  7. Monitor service delivery and advocate for any unmet needs of clients.
  8. Participate in staff meetings and training sessions to enhance service delivery skills.

Qualifications:

  1. A bachelor's degree in a human services field is required, along with QIDP eligibility.
  2. Strong organizational skills and the ability to manage multiple priorities effectively.
  3. Knowledge of the developmental disabilities service system and eligibility criteria.
  4. Ability to collaborate with various stakeholders, including clients and their families.
  5. Self-motivated with the capacity to work independently and as part of a team.
  6. Flexibility to adjust work hours as needed to meet client demands.

Physical Requirements:

  1. Effective communication skills, both written and verbal.
  2. Ability to perform physical tasks such as lifting files and sitting for extended periods.
