Job Summary

Center for Elders' Independence is seeking a highly skilled and compassionate Physical Therapist to join our interdisciplinary team. As a key member of our care team, you will provide restorative and rehabilitative services to our elderly participants, promoting their independence, mobility, and overall well-being.

Key Responsibilities
  • Collaborate with the primary care provider team and interdisciplinary team to evaluate participants' physical abilities and develop personalized plans of care.
  • Provide one-on-one therapy sessions to participants, using a variety of techniques to relieve pain, improve mobility, and enhance functional abilities.
  • Work with vendors to obtain necessary equipment and supplies for participants.
  • Conduct initial assessments and reassessments as needed, documenting participants' progress and reporting to the interdisciplinary team.
  • Supervise and evaluate therapy program aides, providing guidance and support as needed.
  • Participate in interdisciplinary team conferences and contribute to the development of comprehensive care plans.
  • Maintain accurate and timely documentation, adhering to Physical Therapy practice standards and CEI policy.
  • Ensure a safe and clean working environment, adhering to CEI's Safety Policies and Procedures.
Requirements
  • Master's degree in Physical Therapy from an accredited program.
  • Current licensure as a Physical Therapist in the state of California.
  • One year of experience working with the frail elderly population, preferably in a PACE (Program of All-Inclusive Care for the Elderly) setting.
  • Current CPR and First Aid certification.
  • Valid California driver's license and access to a reliable vehicle.
What We Offer

Center for Elders' Independence offers a competitive salary range of $47.76 to $71.62 per hour, based on experience and qualifications. We also offer a comprehensive benefits package, including medical, dental, and vision coverage, as well as a 401(k) retirement plan.
