Job Overview
We are seeking an experienced LPN to provide skilled nursing care and services to patients in their place of residence. The ideal candidate will have exceptional nursing skills, a passion for working with patients in a home setting, and the ability to case manage nursing and non-skilled care and services.

Salary: $65,000 - $90,000 per year

Job Description:
The LPN Assessment Nurse will be responsible for providing skilled nursing care and services to patients in their place of residence. This includes performing health assessments, scheduling and conducting supervisory visits, and providing care coordination. The nurse will also be responsible for preparing necessary items for treatment and procedures, implementing current standards to prevent the spread of infection, and making treatment recommendations based on the patient's acuity level and goal potential.

Requirements:
* 1-2 years of experience as an LPN in a home healthcare setting
* Excellent clinical skills and the ability to manage time well
* Strong communication skills and the ability to communicate effectively with patients, family members, and healthcare professionals
* Ability to follow regulation guidelines and deadlines with submission of clinical paperwork
