Key Responsibilities- Conduct thorough assessments of patients' physical, psychological, social, and educational needs to determine level and type of care.
- Develop and maintain current patient treatment plans to include patient problems, goals, and interventions.
- Deliver care based on acuity level, identified needs, and physician orders.
- Coordinate physical therapy care activities with other departments/disciplines.
- Assess, collaborate, and coordinate patient's discharge planning needs and readiness for discharge.
- Assess patient/family learning needs, abilities, and readiness to learn and integrate needs into plan of care.
- Provide appropriate education/orientation to patients/families.
- Bachelor's degree or post Graduate degree (PT)
- Active Physical Therapist (PT) License in the state of Georgia
- BLS and ACLS Certification
- Minimum 1 year hospital experience, critical care/acute care preferred
- Excellent communication and interpersonal skills
- Superior analytical, problem-solving, and critical thinking skills
- Team, detail, and goal-oriented
- Positive, professional, and empathetic demeanor
