Key Responsibilities:

  • Conduct comprehensive in-home assessments, including medical history reviews, physical exams, and vital sign checks
  • Perform evidence-based screenings and testing to identify potential health risks
  • Develop personalized care plans to address patient needs and improve health outcomes
  • Communicate effectively with patients, families, and healthcare teams to ensure seamless care coordination
  • Stay up-to-date with the latest clinical guidelines and best practices in home-based healthcare

Requirements:

  • Active NP or PA licensure in good standing
  • Current national certification in NP or PA practice
  • Ability to work independently and as part of a team
  • Excellent communication and interpersonal skills
  • Reliable transportation and a valid driver's license
