Job Summary

Burrell Behavioral Health is seeking a skilled Housing Retention Specialist to join our team. As a key member of our community support services team, you will play a vital role in providing additional housing and community support services to clients enrolled in our Comprehensive Psychiatric Rehabilitation (CPR) program.

Key Responsibilities
  • Provide Community Support Services: Work with clients to complete housing stability plans, housing applications, and emergency housing resources to prevent homelessness.
  • Assist with Housing Placement: Collaborate with clients and team members to identify and secure affordable and safe permanent housing options.
  • Support Case Management: Assist primary case managers in providing supportive services that promote housing retention, community integration, life skills, and improved quality of life.
  • Identify and Enroll Homeless Clients: Identify individuals who are homeless or at risk of becoming homeless and ensure they are enrolled in our PATH program.
  • Attend Training and Meetings: Attend all related PATH trainings and community-wide meetings to stay up-to-date on best practices and community resources.
  • Maintain Knowledge of Community Resources: Develop and maintain a good working knowledge of community resources that benefit clients dealing with housing issues and homelessness.
  • Collaborate with Agency Departments: Ensure all agency departments are educated and knowledgeable about PATH funding and referral processes.
  • Document Client Information: Complete all documentation as defined by program, agency, and DMH/Medicaid standards, and ensure timely entry into the electronic medical record.
  • Participate in Team Meetings: Serve as a vital multi-disciplinary team member, facilitating program, departmental, and agency communication and operations.
Requirements
  • Education and Experience: Bachelor's degree in psychology, social work, nursing, counseling, or a closely related human services field. Equivalent experience may be substituted on the basis of one year of experience in case management for each year of required educational training.
  • Preferred Qualifications: Experience working with the homeless population and training or certificates on SOAR, PATH, and Caseworthy.
  • Skills and Abilities: Excellent phone and verbal communication skills, proficient computer skills, crisis intervention, establishing rapport in professional relationships, organization/time management, case management, and knowledge of community resources.
Working Conditions

This is a sedentary position that involves sitting most of the time, with occasional walking or standing. The ideal candidate will have a reliable automobile and adequate auto insurance for frequent home/community visits.
