We are seeking a skilled Physical Therapist Assistant to join our team at AlphaCare Home Health Corp. The successful candidate will provide high-quality patient care in the comfort of their own homes, working closely with our licensed physical therapists to develop and implement treatment plans.
Key Responsibilities:
- Deliver skilled therapy services to patients in their homes, following established treatment plans
- Monitor patient progress and report any changes or concerns to the supervising physical therapist
- Assist in assessing patient functional levels and developing treatment plans
- Complete accurate and timely clinical notes and progress reports
- Participate in care conferences and quality improvement programs
Requirements:
- Current licensure, registration, or certification as a Physical Therapist Assistant in the state of practice
- Graduation from an accredited Physical Therapist Assistant program or equivalent
- Passing score on the national examination for Physical Therapist Assistants
Work Environment and Physical Requirements:
- Ability to communicate effectively and operate necessary equipment
- Sufficient mobility to perform duties and travel to patient homes
- Work in varied environments, including patient homes with different conditions
