About the Role

As a Nurse Care Manager in our Ambulatory Internal Medicine Practice, you will play a vital role in ensuring the overall clinical success of the team. Your primary responsibility will be to support and participate in the Patient-centered Medical Home (PCMH) concepts of team-based care.

Key Responsibilities:

  • Support the implementation of PCMH guidelines and ensure compliance across the team.
  • Assess, plan, implement, coordinate, monitor, and evaluate healthcare options and services to improve patient outcomes.
  • Provide outreach for hospital discharge follow-up, care planning, and coordination of services.
  • Educate Medical Assistants on PCMH requirements and best practices.
  • Collect data on quality metrics and track progress for improvement efforts.
  • Collaborate with patients' medical/health providers to establish mutual goal setting, involving self-management tools.
  • Conduct blood pressure checks for uncontrolled hypertensive patients.
  • Offer education on managing diabetes and achieving good glycemic control.
  • Monitor INRs and promptly inform PCPs when out of range.
  • Facilitate access to medical supplies, including glucometers and nebulizers.
  • Coordinate referrals to homecare services.
  • Assist with unstable patients awaiting 911 arrival.
  • Develop clinical procedures to ensure patient safety, comfort, and quality care.
  • Evaluate ongoing patient care through incident reports and take necessary actions.
  • Serve as a resource and role model for staff.
  • Recommend and implement new initiatives as appropriate.
  • Review insurance registries to address missing gaps.
  • Maintain professional licensure and stay up-to-date on DOH, Medicare, AAAHC, and other regulatory requirements.
  • Establish and maintain effective working relationships with employees, managers, and physicians.
  • Develop patient education materials.
  • Promote a positive image of the practice when interacting with patients and others.
  • Perform other duties as assigned.

Requirements

  • Bachelor's degree in Nursing or equivalent.
  • Minimum 3 years of experience.

Preferred Qualifications

  • Excellent organizational and interpersonal communication skills.
  • Demonstrated ability to manage multiple priorities.
  • Ability to write reports, business correspondence, and letters to patients.
  • Effective communication with groups of patients and physicians.

Estimated Salary Range: $54,630 - $81,940 per year
