Job Summary

We are seeking an experienced Hospice Admission Registered Nurse to join our team at Liberty Hospice. As a Hospice Admission Registered Nurse, you will be responsible for evaluating and admitting patients according to Local Coverage Determinations (LCD) Guidelines provided by the Medicare Administrative Contractor (MAC).

Key Responsibilities

  • Evaluate patients for hospice eligibility and develop individualized plans of care.
  • Coordinate patient care services as necessary, including referrals to other healthcare providers.
  • Document comprehensive assessments and patient information accurately and thoroughly.
  • Work a schedule according to Human Resource Action Form, participate in on-call rotation, and attend monthly staff meetings.

Requirements

  • Current RN licensure within the state of branch location with one year of clinical nursing experience.
  • Successfully completed nursing education from an approved school of nursing.
  • Ability to provide effective healthcare services to patients in their home setting through patient evaluation, care, planning, and coordination.
  • Strong communication, organization, and time management skills.
  • Ability to use a personal computer and have a working knowledge of various computer applications, including Windows, Word, Excel, etc.
  • CPR certification required.
  • Minimum of twelve hours continuing education annually.
  • Valid driver's license.
