Evolve Therapy Services is a leading therapy management organization seeking a compassionate Physical Therapist to join our team at Mount Joseph in Waterville, ME.

Job Type: Full Time, Part Time, or PRN positions available

Job Purpose: To provide high-quality Physical Therapy services in accordance with current applicable federal, state, and local standards, guidelines, and regulations.

Key Responsibilities:

  • Provide physical therapy services to residents in accordance with established standards of practice, Company procedures, and productivity standards.
  • Develop and implement individualized treatment plans to achieve resident goals.
  • Conduct timely screenings of residents to determine need for intervention/treatment.
  • Collaborate with interdisciplinary team members to ensure comprehensive resident care.
  • Document all interactions and interventions in accordance with Company procedures and regulatory requirements.

Requirements:

  • Minimum of Bachelor of Science Degree in Physical Therapy from an accredited program.
  • Active Physical Therapy license in state(s) of practice.
  • Ongoing continuing education and professional development to maintain current licensure and certification.
