Job Summary

We are seeking a highly skilled and compassionate Nurse Practitioner to join our Advantmed provider network. As a key member of our team, you will be responsible for conducting in-home wellness risk adjustment assessments for Medicare and other populations.

Your primary objective will be to assess the overall health and well-being of member beneficiaries to ensure accurate and comprehensive risk adjustment coding, leading to greater value-based care.

Responsibilities
  • Perform annual wellness visits and health assessments on populations with chronic conditions
  • Deliver patient health education opportunities to promote healthy behaviors and disease management
  • Assist in closing quality care gaps, such as screenings and labs, to ensure comprehensive care
  • Collaborate with our team to enhance care delivery and improve health outcomes
  • Commit to a minimum of 30 hours per month to ensure consistent and high-quality service
Requirements

To be successful in this role, you will need:

  • A valid unencumbered NP License for the state you will be working in
  • Previous in-home risk assessment experience is preferred, but not required
  • At least 3 years of patient care experience, preferably in primary care, adult, or geriatric settings
  • Bilingual skills are a plus, but not required
