Job Summary

We are seeking a highly skilled Registered Nurse to join our team as a Home Therapies Nurse. As a key member of our multidisciplinary team, you will be responsible for delivering high-quality patient care and ensuring the effective management of home dialysis programs.

Key Responsibilities
  • Develop and implement individualized care plans for patients undergoing home dialysis.
  • Provide education and training to patients and their families on PD (CAPD/CCPD) and home dialysis modalities.
  • Collaborate with multidisciplinary team members to coordinate patient care and ensure seamless transitions between care settings.
  • Conduct home visits to assess patient needs and provide guidance on dialysis equipment and water quality.
  • Participate in care plan meetings and charting to maintain accurate and complete records.
  • Communicate effectively with patients, families, and healthcare providers to ensure continuity of care.
Requirements
  • Graduate of an accredited Registered Nurse program with current Alaska licensure or ability to obtain.
  • Minimum 1 year of nursing experience, with dialysis experience preferred.
  • BCLS and ACLS certification preferred.
  • Leadership ability, organizational skills, and effective communication skills.
  • Ability to understand and implement government standards.
