Responsibilities- Provide primary and preventive care to patients of all ages, including health promotion, disease prevention, and management of acute and chronic illnesses.
- Conduct thorough health assessments, develop and implement individualized care plans, and provide education to patients and their families.
- Collaborate with our multidisciplinary team to coordinate patient care and ensure seamless transitions between healthcare settings.
- Stay up-to-date with the latest clinical guidelines, research, and best practices in family medicine and primary care.
- Participate in quality improvement initiatives and contribute to the development of policies and procedures that promote excellent patient care.
- Master's degree in Nursing or related field.
- Current licensure as a Registered Nurse and certification as a Family Nurse Practitioner.
- Experience in family medicine or primary care, preferably in an academic or healthcare system setting.
- Excellent communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare teams.
- Strong analytical and problem-solving skills, with the ability to prioritize tasks and manage multiple responsibilities.
