We are seeking a skilled Nurse Practitioner to join our team at Avera, a leading healthcare organization committed to delivering exceptional patient care. As a Nurse Practitioner, you will play a vital role in providing high-quality, patient-centered care in our multidisciplinary team environment.
Key Responsibilities:- Deliver comprehensive, evidence-based care to patients, including assessment, diagnosis, and treatment planning.
- Collaborate with physicians and other healthcare professionals to ensure seamless care coordination and optimal patient outcomes.
- Develop and implement individualized care plans, taking into account patient preferences and values.
- Provide education and support to patients and their families, promoting health literacy and self-management skills.
- Participate in quality improvement initiatives, contributing to the development and implementation of best practices and policies.
- Graduate of an accredited Nurse Practitioner program.
- Current licensure as a Registered Nurse in South Dakota.
- Experience in Family Practice or a related field, with a willingness to train.
- Current certification in ACLS and CPR, or willingness to obtain.
