Job Overview
The Home Health Nurse will be responsible for providing comprehensive care to patients in their homes, working closely with healthcare teams to ensure seamless coordination and excellent outcomes. Responsibilities
* Provide direct patient care in home settings, including wound care, medication management, and education
* Develop and implement individualized care plans based on patient needs and goals
* Collaborate with healthcare teams, including physicians, therapists, and social workers, to ensure comprehensive care
* Conduct thorough assessments of patients' physical and emotional needs, identifying areas for improvement and developing strategies to address them
* Educate patients and families on disease management, self-care techniques, and health promotion strategies

Requirements
* 1 full year of adult Home Health experience or 1 full year Hospice experience required
* Must have recent specialty experience
* Current licensure as a RN (Registered Nurse) in the state of employment
* CPR/BLS certification required
* Ability to work independently, think critically, and prioritize tasks effectively

Compensation
Based on 12-hour shifts and 36 hours per week, this travel nursing assignment offers an estimated salary of $120,000 - $150,000 per year, depending on location and experience. Additionally, Tact Staff provides a tax-free stipend and reimbursement for mileage, making this an attractive option for experienced nurses looking for a challenging and rewarding opportunity.
