Position Overview:

A dedicated registered nurse (RN) who provides personalized support to patients, families, and caregivers to navigate the complexities of the healthcare system. Aims to enhance patient satisfaction and improve clinical outcomes by ensuring a smooth navigation process throughout the care continuum. Responsible for collaborating with physicians and interdisciplinary team members to assess, coordinate, and manage patient care effectively, while educating patients and serving as a reliable point of contact for patients and their families.

Key Responsibilities:

  • Engage in strategic initiatives within the clinical program or service line, including the development, implementation, and monitoring of navigation activities.
  • Facilitate shared decision-making and patient engagement by building trusting relationships and addressing communication and health literacy needs.
  • Assist patients in integrating their values and preferences into treatment decisions.
  • Promote positive outcomes for patients, healthcare providers, and the Sutter health care system.

Qualifications:

EDUCATION:

Graduate of an accredited nursing program.

CERTIFICATION & LICENSURE:

RN-Registered Nurse of California

OR AOCNS-Advanced Oncology Certified Clinical Nurse Specialist

BLS-Basic Life Support Healthcare Provider

EXPERIENCE:

Minimum of 5 years of relevant nursing experience.

SKILLS AND KNOWLEDGE:

Proficient understanding of medical terminology.

Strong ability to follow through with responsibilities and communicate effectively with diverse healthcare staff.

Familiarity with third-party reimbursement processes is advantageous.

Excellent verbal and written communication skills.

Proficient in Microsoft Office applications, including Excel and Word.

Work Schedule:

Full Time, Days, Monday - Friday, 40 hours per week.
