As a key member of our interdisciplinary team, you will provide high-quality patient care to individuals with wounds and ostomies, utilizing the nursing process to optimize outcomes and maximize available resources.
Responsibilities- Develop and implement patient care plans based on comprehensive assessments to achieve optimal patient outcomes.
- Monitor and record patient condition, communicating effectively with healthcare team members to coordinate care.
- Educate patients and families on wound and ostomy care, promoting self-management and independence.
- Collaborate with interdisciplinary team members to ensure seamless patient care transitions.
- Participate in quality improvement initiatives to enhance patient care and outcomes.
- Graduate of an accredited Registered Nursing program.
- State licensure as a Registered Nurse (RN).
- Basic Life Support (BLS) certification.
- Minimum 2 years of nursing experience, preferably in wound care or a related field.
