Position Type: On-site.

Branch Location: Raleigh, NC.

This is not a remote or work-from-home position.

The Clinical Manager is responsible for coordinating and supervising all direct patient care services delivered by clinical staff.

  • Develops, plans, implements, analyzes, and organizes clinical operations for the designated location.
  • Conducts and delegates patient assessments and reassessments, updating care plans and interpreting patient needs while adhering to company and regulatory policies.
  • Manages caregiver assignments and oversees the delivery of care to all patients.
  • Receives case referrals and reviews patient information to determine home health or hospice needs, ensuring patients meet admission criteria.
  • Instructs and guides clinicians to enhance performance and ensure quality home care services.
  • Assists clinicians in establishing both immediate and long-term therapeutic goals, setting priorities, and developing patient Plans of Care (POC).
  • Monitors documentation compliance with regulatory agencies and third-party payer requirements.
  • Facilitates communication among team members, attending physicians, and caregivers to ensure appropriate care and outcome planning.
  • Collaborates with the Branch Director and Finance Department to set revenue and budget goals for the location.
  • Engages in sales and marketing initiatives.
  • Supervises all clinical staff at the location, ensuring effective direction, coordination, and evaluation.
  • Handles employee corrective actions and disciplinary issues in consultation with Human Resources.
  • Participates in the hiring, training, and development of direct care clinicians, evaluating their performance against job goals.
  • Assesses staff educational needs and conducts regular training as necessary.
  • Analyzes situations to identify problems and evaluate alternative solutions using Performance Improvement principles.
  • Reviews clinical staff documentation for compliance with care delivery and documentation standards.
  • Responsible for Quality Assurance/Performance Improvement activities, collaborating with Utilization Review staff for data tracking and performance analysis.
  • Provides direct patient care only in emergencies.
  • Acts as Branch Director in their absence.
  • Ensures compliance with company standards and external regulatory authorities.
  • Maintains ongoing clinical knowledge through training programs and shares insights with staff.
  • Builds relationships with referral sources and participates in professional organizations.
  • Performs other related duties as assigned.

Required Experience/Skills:

  • Graduate of an accredited School of Nursing.
  • Current state license as a Registered Nurse.
  • Proof of current CPR certification.
  • Valid driver's license and reliable transportation.
  • Minimum of two years as a Registered Nurse with at least one year of management experience in home care or hospice.

Additional Information:

  • Experience with Homecare Homebase (HCHB) software and OASIS knowledge is highly preferred.
  • Strong clinical background in home care operations and quality assurance is preferred.
  • Clinical leadership experience managing a team of 15+ direct reports across various disciplines.
  • Normal Hours of Operation: Monday to Friday, 8 AM to 5 PM (ET).
  • On-Call Expectation: Yes, occasional on-call shifts required.
  • Branch Size: Medium.
  • Bonus Eligible: Yes, eligible for quarterly and annual incentives.

Scheduled Weekly Hours: 40

Pay Range: The compensation range reflects a good faith estimate of starting base pay for full-time employment. The pay range may vary based on location and individual qualifications: $76,800 - $105,800 per year. This position is eligible for a bonus incentive plan based on performance.
