We are seeking a highly skilled and compassionate Case Manager to join our team at Westhab, Inc. As a Case Manager, you will play a critical role in assessing clients' needs, developing individualized service plans, and linking clients to essential services and resources.
Key Responsibilities- Conduct thorough intake and assessment processes to orient clients to our program and develop tailored service plans.
- Meet regularly with clients to support the achievement of their service plan goals and objectives.
- Collaborate with Housing Specialists, other staff members, and external partners to coordinate services and ensure seamless support for clients.
- Provide advocacy and support for clients in accessing entitlements and services, as well as crisis intervention and counseling services.
- Document case management activities in accordance with agency and funding source guidelines and procedures.
- Complete housing applications for clients based on their individual needs and circumstances.
- Achieve monthly placement quotas, as assigned.
- Facilitate workshops, groups, and escort clients as necessary.
- Participate in staff meetings and training sessions to enhance skills and knowledge related to job functions.
- Work flexible schedules, including evenings and weekends, as required.
- Master's degree in Social Work (MSW) or a related field, or a Bachelor's degree in Social Work or Human Services with 5 years of case management experience working with vulnerable populations.
- Bilingual skills a plus.
- Strong relationship-building skills, with the ability to work independently and collaboratively as part of a team.
- Excellent organizational and time management skills, with the ability to multitask and prioritize tasks effectively.
- Strong PC skills, including proficiency in Microsoft Office and Excel.
