**Job Description:**

The Housing Resource Navigator (Case Manager) is responsible for providing essential services to families and individuals in need of housing and support. These services may include providing general information about various housing programs, crisis intervention services, developing action plans, providing an appropriate level of guidance and support, facilitating referrals, and serving as an advocate on behalf of participants to assist them in securing services and support to reach their housing goals.

**Responsibilities:**

  • Assess displaced client and family needs out in the field
  • Meet with clients at their home or where they are currently residing
  • Develop comprehensive housing plans for families to become self-sufficient
  • Coordinate needed services
  • Develop links with a continuum of services and agendas
  • Maintain up-to-date client records

**Requirements and Qualifications:**

  • Minimum of 2 years' case management experience; knowledge of homelessness, addiction and mental health issues as they pertain to provisions of supportive service
  • Demonstrated communication skills and sensitivity to various cultures and socio-economic groups
  • Working knowledge of computer skills, including proficiency with Microsoft PowerPoint, Word and Excel

**Additional Information:**

  • Able to pass a pre-employment drug test
  • Must be able to sit, talk and listen, stand, walk and use hands and arms to use a keyboard, handle papers, and reach for the phone
