Job Summary

We are seeking a highly skilled and compassionate Nurse Practitioner to join our Primary Care team at CVS Health. As a key member of our clinical care team, you will provide high-quality, patient-centered care to a panel of patients, working collaboratively with a dedicated team to drive better outcomes.

Responsibilities
  • Provide accurate and complete assessments, diagnoses, treatments, and management of health problems, health counseling, and disposition planning.
  • Collaborate with the clinical care team to ensure the delivery of high-quality, patient-centric, longitudinal care.
  • Communicate effectively with members of the clinical care team, other health professionals, and health-related agencies.
  • Participate in and lead clinical care team huddles, promoting mutual respect and trust within the team.
  • Manage clinical and non-clinical duties efficiently, adapting quickly to new models of patient care for clinic efficiency.
  • Enhance operational effectiveness, emphasizing cost containment without jeopardizing important innovation or quality of patient care.
  • Assist with hiring, development, and evaluation of Primary Care clinical care team members as needed.
Requirements
  • Master's degree in Family Nurse Practitioner with current National Board Certification and State of Employment license to practice in the Advanced Practice role.
  • A minimum of 1 year experience as a Nurse Practitioner, with experience in Family medicine or primary care practice preferred.
  • Active Advance Practice Registered Nurse, Nurse Practitioner license in good standing.
  • Current National Board Certification in Family Practice.
Working Environment

The Primary Care NP will work in a dynamic and fast-paced environment, interacting with patients remotely, sitting, writing, operating the computer and phone, speaking intelligibly, and hearing patient responses. Specific vision abilities include the ability to view and read a computer screen and other electronic devices.
